Caring for the emotional needs of nursing home residents is as important as caring for a resident’s physical needs. The team at Grande Oaks has made that its mission to understand. The staff understands that the decision for nursing care is not something that is entered into easily by most residents and their staff. Over the years, the facility has had an extremely dedicated staff who are trained to assist in reducing anxiety involved as a person transitions from home or hospital to a long-term care facility. At Grande Oaks, resident focused care is at the heart of what the dedicated staff offers. We understand that each resident is unique, and thus, staff caters to each resident’s individual needs. Whether it is our fabulous activities department providing therapeutic recreational programs, the therapy department that always goes the extra mile in helping our residents improve their quality of life, or our Nutrition department that makes sure that our residents have a diet that uniquely meets their personal needs, the staff at Grande Oaks is committed to helping each resident reach his or her highest level of functioning.

Grande Oaks accepts Medicare and Medicaid, as well as most major insurances. We offer private and shared suites. We have an experienced Board Certified medical director as well as an additional professional healthcare team that includes 24-hour Respiratory Therapists in our pulmonary unit, Optometry, dentist, podiatrist, mental health staff, and other specialties.

Short Term Skilled Nursing Care

At Grande Oaks we offer a vast variety of services for all patients, and all types of acuity of care. Our goal for our short-term rehabilitation is to get the patient the best care they need, and to get them back to their homes, safe and in the quickest time possible.

In our smaller, home-like, patient centered environment we offer care for many different diagnosis and issues along with our 7 day a week therapy. Just a few of the services and diagnosis that we provide care for are as follows:

  • Ventilator Care - Respiratory Therapists on staff 24 hours a day, 7 days a week
  • Wound Management - On-staff Wound Nurse
  • Stroke Post - Acute Recovery Care

  • Congestive Heart Failure Post-Acute Recovery Care
  • Pulmonary Care Including Pneumonia treatment and COPD programming

Long Term Care

When available, Grande Oaks in Oakwood Village offers the most affordable long-term care options to all residents of the community. Some folks may just be ready for a higher level of care from home, or just may realize they cannot return to their home. We take what we do serious, and we want everyone to be a part of our FAMILY.

Ventilator Care

What sets Grande Oaks apart from all other Skilled Nursing Facilities, beyond the fantastic care you or your loved one will receive, is our Ventilator Care Unit. Grande Oaks is one of the very few centers in the state of Ohio to service our Ventilator community. Our Ventilator Care Unit at Grande Oaks is here to serve our patients and give them not only the best care, but to give them an enjoyable way of life with support, and daily activities from our lovely, experienced staff.

Specialized Care Offers:

  • Unlike most centers Grande Oaks offers Respiratory Therapists on staff to our residents 24 hours a day, 7 days a week
  • Separate Ventilator unit with Private rooms
  • In-House physician to monitor respiratory needs and any other needs a resident may have
  • Regular Care Conferences with the interdisciplinary team offering insight, and updates on every individuals care
  • Individualized Respiratory Care Planning
  • Interdisciplinary team that includes speech, physical, occupational and respiratory therapists, as well as consultant pulmonary specialists when needed
