Crystal Care Center of Ashland

Crystal Care Center of Ashland has a long history in the Ashland area. Located at the corner of Main Street and Route 250, just minutes away from the thriving downtown district. We are centrally located between Mansfield, Medina, and Wooster

Our small, home-like environment features 72 rooms, and recent updates to the exterior perfectly complement previous updates to the building’s interior. We are also a 2019 Deficiency Free Survey recipient from the Ohio Department of Health.

We provide top-notch therapy services, including speech, physical and occupational therapy. We also provide medication management, pain management and Alzheimer’s and dementia care. Our Board-Certified Medical Director and Nurse Practitioner are in the building regularly.

Short-Term Care

Crystal Care Center of Ashland provides top-rated respite care for those family caregivers who need a break or a vacation. You can rest assured that your loved one will be in a deficiency-free environment when they stay here. Our hospice services are contracted with several local companies.

Our trained staff is adept at IV management, tube-feeding management and colostomy/ostomy care. We also provide wound and skin management with our wound care certified nurse. Restorative therapy is available for those discharged from skilled services to ensure success at home

It’s our mission is to know you and your family and provide any necessary support. All this information will help us to provide a Person-Centered Care Plan for each person coming through our doors. We accept Medicare and Medicaid and other major insurances.

What is Empower Outpatient Therapy?

Empower Outpatient Therapy allows people to receive therapy at one of our local clinics, in their own community.  We often treat patients with similar medical conditions as those in an inpatient setting. The difference is that patients visiting our Empower Outpatient clinic tend to have conditions that, while acute in nature, are less medically complex and with fewer comorbidities. For instance, a first-time stroke patient with no other medical history or complications may get treatment in an outpatient clinic, while a patient dealing with multiple strokes and other medical issues would more likely be treated in a post-acute setting.

Empower Outpatient therapy is perfect for those who need help to restore their health after experiencing minor injuries or other health conditions. The services you can receive from outpatient programs generally include Physical Therapy, Occupational Therapy, and Speech Therapy. Our clinics are ideal for patients looking to improve their daily mobility and quality of life, but are independent enough to receive these services while living in their home or community. We focus on helping patients restore function following an injury, so they can get back to their normal activities.

Our clinics may also provide other types of services, such as pre-or post-surgery rehab, biofeedback, and a variety of modalities including ultrasound, diathermy, and more. We are a great option for services that aren’t offered within the community such as voice or speech therapy. Visits to an outpatient clinic tend to be 45-60 minutes in length, and patients often continue their exercises at home between sessions. Medicare patients may be eligible to receive outpatient therapy under Medicare Part B and, because the therapy caps were repealed, they can benefit from improved access to these services.

How to get started with Empower Outpatient Therapy?

To get started in the program, outpatient orders from your physician are initiated, insurance is verified and your initial evaluation is scheduled. After the initial evaluation, a licensed therapist will outline your individualized plan which includes treatment goals, therapeutic interventions, and the frequency/duration of care. Pulmonary Care

At Crystal Care Center of Ashland, we understand having a Pulmonary Unit with a dedicated staff can drastically reduce rehospitalization, allow for a faster recovery, and foster a better quality of life for our patients and residents. Our Pulmonary Care Program, EMPOWER PULMONARY, strives to help our short-term and long-term residents with diagnoses of COPD, pneumonia, asthma, or respiratory failure re-establish their highest functioning pulmonary status.

Wound Care

Embassy's Wound Care Program is led by Dr. Marc Berkowitz

Dr. Marc A. Berkowitz started practicing Podiatric Medicine and Surgery in the greater Cleveland area in 2007. Dr. Berkowitz has special interests in Wound Care, Reconstructive Foot Surgery, Sports Medicine and Diabetic Foot Care. Dr. Berkowitz graduated from the Ohio College of Podiatric Medicine in 2004 where he served as an adjunct clinical faculty member. He completed a three year surgical foot and ankle residency at University Hospitals Health System/Richmond Heights where he served as a clinical attending. During his post-graduate residency, Dr. Berkowitz received specialized training in Pediatric Lower Extremity Surgery in Vienna, Austria. Dr. Berkowitz currently owns and operates Wound Care Consultants LLC, a multi-specialty company composed of physicians and nurse practitioners that render conservative and invasive care to patients residing in post-acute type facilities. Dr. Berkowitz is double American Board Certified in Podiatric Medicine and Wound Healing.

What is Wound Care?

What is the Wound care program? 
The wound program would be more of a preventive program to start.  Reviewing new admissions/ being proactive with current residents in identifying nutrition and surface selection for the residents. Weekly rounds and communication between the facility and the wound provider.   Weekly and monthly outcome tracking.

Why is it a good program?
Our wound program is good because we focus on being proactive with new and current residents to help reduce the chances of new wounds to develop. When wound care is needed the physicians look at the entire care of the residents and address them holistically with the IDT.

What are the benefits of the program?
Staff and resident education by the provider. 7 day provider available to assist with complex wounds on admission/ skin sweeps. Advance skin substitutes for wound healing.

Kenneth Arsham, M.D.

Kenneth Arsham M.D was born and raised in Cleveland, Ohio.  He earned his medical degree at Georgetown University, in Washington D.C in 1979 and completed a General Surgery residency and Plastic Surgery fellowship at University Hospitals of Cleveland.

Dr. Arsham received specialized training in Laser surgery in the 1980’s and taught nationally.  Wound Care was always part of his practice and after further wound training from Healogics in 2009.  He became actively involved in the South Pointe Wound Care Center.  Also, he speaks nationally on wound healing on behalf of Smith-Nephew.  He has served as a wound care provider for Wound Care Consultants, LLC since 2019.

Dr. Arsham is the past chief of the Department of Surgery at South Pointe Hospital, a Cleveland Clinic Hospital and has been the chief of the Division of Plastic Surgery since 1990.  He has been certified by the American Board of Plastic Surgery since 1987.

Rachel Askew, FNP-BC, WCC, Nurse Practitioner

Rachel Askew, FNP-BC, WCC, is a Family Nurse Practitioner who specializes in wound and ostomy care with Dr. Marc Berkowitz at Wound Care Consultants, LLC. She completed her Bachelor of Science in Nursing at the Ohio University in Athens, and then earned her Master of Science degree at Kaplan University in Chicago, Illinois. She holds her national board certification with the American Nurses Credentialing Center (ANCC) as a Family Nurse Practitioner. Rachel has more than 15 years of nursing experience in acute, subacute, and outpatient care settings. Rachel has a passion for wound care, healing hands, is a member and board certified in wound care through the National Alliance of Wound Care and Ostomy (NAWCO).

Shari Villafane, Nurse Practitioner

Shari Villafane, a board-certified nurse practitioner, has lived in the Cleveland area for thirteen years. She graduated with her master’s degree as a Family Nurse Practitioner in 2018 from Chamberlain University. Shari’s clinical rotations were spent in a family practice, a pediatrics’ office, and in an urgent care. Shari has been practicing Urology for two and a half years. Shari has more than ten years of experience in the medical field. Prior to urology, Shari worked in Women’s health for 4 years as nurse and sent the time as a member of the research team. Shari also spent time as a nurse in the float pool.
