Located just off the coast of Lake Erie, Madison has access to Downtown Madison Village, Lake Health Emergency Rooms, and Powderhorn Golf Course. Our 125-bed facility boasts long-term care and skilled nursing capabilities as well as memory and behavioral care. Madison has seven private suites and semi-private, triple, and quad rooms.

Skilled Nursing
Our newly-renovated skilled nursing facility is expertly equipped to ease your short-term stay following an injury or illness. Our skilled nursing staff will be able to assist you with everything from wound care to physical rehabilitation. Therapy services are provided through our partnership with Aspen Rehab.

Memory Care
Of Madison’s 125 beds, 39 of them are available in a secure memory unit. Our staff oversees Alzheimer’s and dementia care as well as mild behavioral management. We maintain an activities department with staff onsite daily to engage with our residents. Outings include concerts, parks, and trips to the museum.
